Prepare For Your Office Visit
Temporarily, our doctors are seeing many patients via telemedicine, offering a real-time, interactive audio/video session over a smartphone or computer. Telemedicine provides the same experience as an in-office visit without risking COVID-19 exposure to you or our staff. Learn more about telemedicine.
For an in-person office visit, please bring:
- All completed intake forms and paperwork provided to you by our office.
- Your current insurance information/insurance card.
- A current list of your medications and doses.
- Pertinent medical records. A few days prior to your appointment, check with your referring provider’s office to ensure that your records have been sent to us via fax: 541-685-5920.
We ask that you arrive early to allow time to park and locate our office. We do our best to accommodate every patient to the best of our ability; however, if you are late for your appointment, we may have to reschedule you for another day.
